If you apply for a new voter ID card for yourself or for your family member you can do it online. If you going to apply for a voter id card online you need these three documents first. You must prepare these three documents and then you can apply.
1. Your Passport Size Photo
2. Age proof
3. Address Proof
1. Your Passport Size Photo:

2. Age Proof:
Date of Birth Details(Supported formats .jpg,.jpeg)(max. 5MB)
- List of Supporting Documents.
- Birth certificate issued by competent Local Body/Municipal Authority/Registrar of Births and Deaths
- Indian Passport
- PAN Card
- Driving License
- Aadhaar Card
- Certificates of Class X or XII issued by CBSE/ICSE/State Education Boards, if it contains Date of Birth
3. Address:
Upload a self-attested copy of address proof either in the name of the applicant or any of the parents/spouse/adult child, if already enrolled as an elector at the same address(Supported formats .jpg, .jpeg)(max. 5MB).
How to apply:
Login to https://nvsp.in.
Click the Login(or)Register button.
If you already have the account means you can log in with your username and password. You did not have the login detail just create an account.
After login to the account click home link at the top of the page to and select Register as a new Elector/Voter option.
It will take you to the next page. In that, you will select the Form 6 link I show in the below screenshot.
If you want to change the Registration Form language you can change it on top.
Now you landed on the Registration page here enter your information like Name, Address, Photos, Age Proof, Address Proof, District, State, Assembly/Parliamentary Constituency, etc.
You enter your information in both English and your state language.
After filling the all the information click the Review and Submit button to submit the form.
